Eventually someone called them and said to skip it if they dont have one and that the company was still using the old forms...

and that "required" was removed off the application. It took almost 2 weeks for someone to call and tell them that. Customer service (which is nothing but a call center that reads a script) and they couldnt answer "its required". The managers work at the same call center and couldnt answer the same. Finally someone called from the insurance company to tell them "oops".
